An unannounced monitoring visit and facility tour was conducted from 10:30am-12:10pm. At the time of inspectors' arrival there were 92 toddler to school aged children in care with 12 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, lunch, outdoor play, transitioning between activities and toileting and hand washing. A sample of 9 children's records and 9 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the center administration at the conclusion of this inspection and are detailed on the violation notice.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview it was determined the center did not ensure that each staff record contained all of the required components. Evidence: 1. The record for staff #7(hired 2/24/19) did not contain the two required references. . 2. The record for staff #8(hired 3/ /19) did not contain: -the date of hire -one of the two required references -documenation of orientation training Plan of Correction: Documentation will be requested from staff and added to records once received.
