Comments:
An unannounced complaint investigation was conducted today. The Inspector completed staff interviews and a review of administrative documents today. In addition, the Inspector completed a monitoring Licensing inspection today. Children's records, staff records, emergency preparedness items, staffing, the physical plant, injury reporting, and children's nutrition were inspected today. There were 59 children in care, with a total of 14 staff members assigned to their direct supervision. After careful review of all available information, this complaint was deemed not valid. However, violations associated with the monitoring inspection were cited. The children were observed enjoying circle time discussions, listening to stories, creating theme-related art projects, playing outside, eating lunch, and taking their afternoon nap. The children were served burritos, oranges, corn, and milk for lunch today. This inspection was completed between approximately 9:40am and 2:00pm. Thank you for meeting with me today. Description: Based on a review of recent injury reports, it was determined that one report was not completed as required. Evidence: One injury reported completed on 4/25/19 does not include two staff/parent signatures.

Plan of Correction: A management staff person signed the report but the parent refused to sign. Moving forward, all staff will be instructed to get two staff signatures if a parent refuses to sign an injury report.

Description: Based on observations, it was determined that staff did not sit with children during meal time.
Evidence:
In the following classrooms, staff were observed not sitting with the children during lunch: Preschool, Toddlers, Two's.

Plan of Correction: All staff will be reminded to sit with the children during lunch.
