Comments:
A announced monitoring inspection was conducted with the director and one staff. Four children were in care with an age range of 11 to 14 years old. Children were observed during homework time and snack time. No children were on medications and all required paper work was posted. Four children's and two staff records were reviewed. Standard #: 22VAC40-185-260-A
Description: Based on review of the annual inspection not all were complete. Evidence: The date on the annual fire inspection is 4/3/18.

Plan of Correction: We will ask the school for the updated fire inspection.

Standard #: 22VAC40-185-550-E
Description: Based on review of the monthly evacuation drills the record was not maintained. Evidence: There was no documentation of a escape practice for the month of April 2019.

Plan of Correction: We will document all evacuation drills.
