Comments:
An unannounced monitoring visit was conducted today beginning at 7:30am. At the time of entrance, 16 children were in care with 3 staff members present. The physical space, evacuation drills, attendance records, medications, 5 children's records and 3 Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The site was clean and organized. The children were observed checking in, playing with toys, reading, and lining up to go outside. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. A loveseat near the desk had red duct tape across a large area to cover tears in the seat.

Plan of Correction: I will get rid of the chair and order a new one.
