Comments:
An unannounced renewal inspection was conducted today from 2:45pm-4:00pm. There were 26 school age children directly supervised by 2 staff. The physical plant, 3 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating kickball, basketball and checking into the program. There was asupply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete records for each enrolled child. Evidence: Child 2 and 3 are missing one parent work phone number.

Plan of Correction: We will get the parent work information that is missing.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, all staff did not submit to fingerprinting. Evidence: Staff 1 did not submit to fingerprinting.

Plan of Correction: Staff have been scheduled to get fingerprinted but have been cancelled and rescheduled by fieldprint. The staff member has an appointment on May 21, 2019.
