Comments:
An unannounced monitoring inspection was conducted today from 10:30am-3:30pm. A total of 129 children were observed in the care of 28 staff. Observations were conducted during infant feeding and diapering, gross motor activities and play in the Movement Zone, handwashing, lunch, storytime and transitions to nap. A sample of 12 staff records, 10 children's records, postings, medications and other documents were reviewed. Areas of non-compliance are documented on the violation notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a sample of records reviewed, 2 out of 12 staff records did not include documentation of a tuberculosis screening within 21 days of employment or within 12 months prior to employment. Evidence: There was no documentation of a tuberculosis screening available for Staff #2 [date of employment: 2/11/18]. A tuberculosis screening for Staff #4 [transferred date: 9/18/2018] was obtained in 2006.

Plan of Correction: We will have the staff obtain tuberculosis screening results.

Standard #: 22VAC40-185-60-A
Description: Based on a sample of records reviewed, 1 out of 10 children's records did not include the required information. Evidence: The record for Child #10 was missing the name, address and phone number of 1 of 2 required emergency contacts.

Plan of Correction: We will obtain the missing information from the parent.

Standard #: 22VAC40-185-70-A
Description: Based on records reviewed, 1 out of 12 staff records did not include the required information. Evidence: The record for Staff #4 was missing documentation of at least two references prior to employment and documentation that the staff possess the experience required by the job position.

Plan of Correction: We will obtain the information missing in the staff record.

Standard #: 22VAC40-185-510-A
Description: Based on records reviewed, an authorization was not provided for a nonprescription medication. Evidence: A nonprescription medication for Child #4 was observed on site; however, an authorization was not available.

Plan of Correction: We will obtain the form from the parent.

Standard #: 22VAC40-185-510-J
Description: Based on observations, medications were not stored in a locked place. Evidence: 1 over-the-counter medication and 1 prescription medication belonging to Child #4 were observed in a storage cubby in an infant room.

Plan of Correction: Corrected during inspection; Medications were moved to a locked cabinet.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a sample of records reviewed, the records for 5 out of 12 staff were missing results of central registry from another state in which the individuals have resided within the past five years. Evidence: 1. A sworn statement signed by Staff #1 [dated: 3/12/2019] disclosed residence in Maryland within the past five years. There was no documentation of out of state central registry results available at the time of the inspection. 2. A sworn statement signed by Staff #2 [dated: 12/10/2018] disclosed residences in New York and Florida within the past five years. There was no documentation of out of state central registry results for either state available at the time of the inspection. 3. A sworn statement signed by Staff #3 [dated: 2/25/2019] disclosed residence in the state of Maryland within the past five years. There was no documentation of out of state central registry results available at the time of the inspection. 4. A sworn statement signed by Staff #4 [dated: 9/18/2018] disclosed residence in California within the past five years. Staff also disclosed current residence in Maryland. There was no documentation of either out of state central registry results available at the time of the inspection. 5. A sworn statement signed by Staff #6 [dated: 1/24/2019] disclosed residence in the state of Maryland within the past five years. There was no documentation of out of state central registry results available at the time of the inspection.

Plan of Correction: We will submit the requests for out of state central registry background checks.
