An unannounced Renewal inspection was conducted in the Center on May 7, 2019. As reported, there are 59 children currently enrolled in the program. There were 41 children in attendance at the time of the inspection. The ages of the children ranged from 2 years to school-age. Upon arrival in the center, there were 18 preschool-age children observed in three groups. Twenty three school-age children were transported to the Center after school for afternoon care. The school-age children were observed in one group. The LI observed the children during free play and a group of pre-K children were observed rehearsing for their upcoming graduation. Hand washing procedures , afternoon snack and dismissal at the end of the day were also observed. A sample of five children's records was reviewed. A sample of complete staff records and background checks were also reviewed. Documentation of the annual fire and health inspections were reviewed and current. Medications were observed in the center. A discussion was held with the ED regarding practiced emergency drills. Each type of drill shall be completed and documented independent of each other. An evacuation drill is to be practiced monthly; a shelter-in-place drill is to be practiced at a minimum of two times a year. The inspection was conducted from approximately 2:10 to 6:20 PM. Standard #: 22VAC40-185-510-C Description: Based on a review of medications in the center, the Center failed to ensure the duration was indicated on the written authorization from the child's physician for long term use. Evidence: The written authorization for administration of prescribed medications to child a. did not include the duration of the physician's authorization for long term use. The written authorization was signed by the physician and dated 06/12/18. Plan of Correction: The parent will be notified and requested to obtain a current written medication authorization from the child's physician to include the duration of the authorization.
Standard #: 22VAC40-185-510-G Description: Based on a review of prescribed medications and staff interview, the center failed to ensure administered medications were dispensed from a pharmacy and maintained in the original labeled, container. Evidence: Prescribed medications for child a. were observed in a medication container from a pharmacy. The annual dates that the prescription was filled and the date to discard had passed, 01/2018 to 01/2019. 2. Upon interview, staff reported that the child's parent brought in extra medication from home and added it to the container that was kept in the center. 3. The reviewed medication record indicated that staff had administered the medication to the child. Plan of Correction: The center will ensure medication policies and procedures are followed. The Executive Director will notify the child's parent and request that a current prescription dispensed from the pharmacy and maintained in the original container is provided to the center for administration.
