Comments:
An unannounced monitoring inspection was conducted from 9:40 am-12:00. There were a total of 77 children with 14 staff members present. Children were observed engaged in group activities and outdoor play time. The playground and surfacing was inspected. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground, the resilient surfacing does not comply with minimum safety standards . Evidence: The shredded rubber surfacing does not encompass sufficient area underneath and surrounding the Little Tikes climber. The climber is located in the middle of the playground. The ground is bare at the bottom of the two blue slides and around the slide.

Plan of Correction: We will get more surfacing.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation of the older infant classroom, the ratios of one staff for every four children is not being met. Evidence: The older infant classroom was observed around 10:00 am, there were 2 staff with 9 children, the youngest child being 8 months old. Staff stated that 2 children were from the younger infant classroom. The younger infant classroom had 4 infants with one staff.

Plan of Correction: We will be hiring 3 more staff.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children medications, the center's procedure for administering medications does not include Include duration of the parent's authorization for medication. Evidence: Child #1 and child #2 each had 2 medications at the center. There was an allergy action plan and emergency care plan for these medications but no medication authorization form to include the duration of the parent's authorization.

Plan of Correction: I will discuss with owner about using the model medication form provided today.
