Comments:
An unannounced mandated inspection was completed on 05/06/2019 from 9:40 A.M.-2:15P.M. At the time of the inspection 55 children were in care with eight staff and two administrators present. Five children records, seven staff records, five injury reports, five medications and required documentation, health inspection, fire inspection, emergency plan, evacuations drills, first aid kits, emergency supplies, playground, injury prevention plan, playground plan, and posted required information were reviewed. Children were observed during handwashing, snack time, transitions, behavioral guidance, various Mother Day art activities, free choice activities that included building, coloring, dramatic play, outside gross motor play, lunchtime, and naptime. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that all equipment was maintained in a safe and operable condition. Evidence: On the playground the school age climber on the second platform there is an area near the entrance of the slide that has peeling paint and is rusting.

Plan of Correction: Administration will work with maintenance and have the area scraped, sanded, and repainted.

Standard #: 22VAC40-185-520-C
Description: Based on a review of diaper ointment, the center failed to label all containers with the child's name. Evidence: 1. In the boy's bathroom located off the kitchen there was a container of Desitin Diaper Ointment that was not labeled with a child's name. 2. Staff verified that the diaper ointment was not labeled with the child's name.

Plan of Correction: Administration immediately labeled the diaper ointment with the child's name. Administration will review over the counter medication requirements with the staff.
