Comments:
The inspector conducted an unannounced mandated inspection from 10:00 a.m. until 11:30. There were 22 children in care with staff supervising at all times. Children were observed playing at centers, having snack, and and making mother's day cards. Five children's records were reviewed and five staff records were reviewed. The violation is identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain the playground in a safe condition. Evidence: 1. The inspector observed multiple large pieces of wood mulch (4 inches long) on the playground.

Plan of Correction: Center staff will pick up pieces and collect them and/or break them.
