Comments:
An unannounced monitoring inspection was conducted today from 10:20am-11:45am. There were 39 children directly supervised by 9 staff and parent volunteers. The physical plant, 3 staff records, 5 children?s records, 5 parent volunteer records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, cleaning up learning centers, eating parent provided snacks of apple sauce and pretzels, and making Mother's day gifts. Staff interactions were observed to be kind and promote cooperation and self expression. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five children's records was incomplete. Evidence: Child 3's record did not contain the name or contact information for one of the two required emergency contacts.

Plan of Correction: We will ask the parent to provide us with the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in a safe condition. Evidence: The plastic sink insert for the dramatic play kitchen in the "Grasshoppers: classroom has a broken corner which is sharp and a finger entrapment hazard.

Plan of Correction: We will repair the sink.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: An unlocked cabinet in the "Bees" classroom contained: bleach, bleach spray, and a 6 inch knife. Children were not present in the classroom during the observation.

Plan of Correction: The cabinet has a lock on both sides. During the inspection the director locked the cabinet and spoke with the staff in he room to remind them to lock the cabinet.
