Technical Assistance:
Reminder to submit the license renewal prior to the expiration of the current license (August 5, 2019).

Comments:
An unannounced follow-up inspection from the license issuance was completed today. The physical plant, 5 staff files, 5 children's files, children's activities, emergency procedures, drills and supplies were inspected between 11:10am-12:40pm. There were 11 children under the supervision of 3 staff. Today, the children ate lunch, cleaned up, sang songs and danced to music, had circle time with a special visitor (fire department official) and talked about their fun plans that they have for this weekend. The children bring in their lunches and/or snacks from home each day they are in care. All of the staff are certified in MAT (medication administration training). Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 5 children's records does not have all required information. Evidence 1. Child #2 does not have the addresses of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: We will ask the parents to complete this.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 3 out of 5 staff records do not have all required information. Evidence 1. Staff #2, #3 and 5 do not have written information to demonstrate that the individual possesses the education required by the job position.

Plan of Correction: The Director asked the staff to bring this in to keep at the center. She will ask them again.

Standard #: 22VAC40-185-550-M
Description: Based on file review and interview, the center has not maintained a written record of minor injuries in which entries are made the day of occurrence. Evidence 1. Since the center began operating, there has not been any documentation of a minor injury sustained by the children in care. 2. One of the teachers informed the LI that there have been a few bumps on the head or small injuries, but that no written record was maintained.

Plan of Correction: Any minor injuries the children sustain while in care will be documented.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on file review, an applicant does not have the findings of a search of the central registry (SCR) on file. Evidence 1. Staff #2 does not have the results of a SCR on file.

Plan of Correction: The Director completed this, but cannot find the results of the search. She will continue to look for this. If it is not found in a week, she will complete it again.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations and file review, the findings of the most recent inspection were not posted on the premises of the facility. Evidence 1. The most recent Violation Notice was kept in a binder.

Plan of Correction: Today's inspection will be posted.
