63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on May 9, 2019, at approximately 9:00 am - 1:30 pm. Observations included indoor areas, program, supervision, medication, health and safety, staff records and children's records. Upon entrance there were 45 children present and 9 staff members. Additional staff were on site for administrative and support purposes. Six staff records , six children's records and three medications were reviewed. The children were observed during morning snack, outside playing on the playground, story time, arts and crafts, play doh activity and nap time. Exit interview conducted with program director.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review, the licensee did not ensure to obtain copy of results of a search of the central registry maintained by any state which the individual has resided in the past five years. Evidence: Based on 1 of records reviewed , staff #1 ( DOH -03/18/2019) did not have documentation of central registry from previous states or information that the state was contacted. Plan of Correction: Director sent in the request for staff #1's. Background check on 5/9/19 and sent copy to licensing inspector.
