Technical Assistance:
**Discussed with the site director to ensure that BOTH parents are documenting their work numbers if they are employed and working. If they do not have a work number or work for a government agency that requires that number be kept confidential, please have the parents document that in the child's records. Discussed circumstances in which only one parent is required to be on record.

Comments:
An unannounced Renewal Inspection was conducted today from 6:16am to 7:40am. There were 5 school-agers directly supervised by 2 staff. The physical plant, 3 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care, participating in group and individual activities and playing in the gym. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-40-J
Description: Based on review, the facility did not ensure that there were documenting annual reviews or updates of the injury prevention procedures, based on injuries and a review of the activities and services. Evidence: The facility did not have documentation of the annual review or update of the injury prevention procedures.

Plan of Correction: Will complete by the end of the day.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that CH1, CH4 and CH5 did not have documentation of work telephone numbers for each parent.

Plan of Correction: Will updated children's records by the end of the day.

Standard #: 22VAC40-185-240-C
Description: Based on review, the facility did not have documentation that program directors and staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development. Evidence: Reviewed 3 staff records and found that Staff 3 did not have documentation of 16 clock hours of annual training.

Plan of Correction: Will obtain documentation of 16 hours of training by the end of the day.

Description: Based on review, the facility did not ensure that written injury reports were maintained with the required documentation. Evidence: Reviewed 5 injury reports and found that 3 did not have documentation of the time and or date of when parents were notified of the injury.

Plan of Correction: Will updated injury reports by the end of the day.
