Comments:
An unannounced monitoring inspection was conducted from 8:50 am-12:40pm. At the time of entrance, 63 children were in care with 25 staff members present. Children were observed during breakfast time, making sticker art, listening to the teacher read a book, bathroom breaks, washing hands, playing at the table with toys, playing in the play gallery, diaper changes, listening to the teacher sing to them, being held while drinking a bottle, group activity cutting an apple, playing at the water table and various centers. The interactions between the children and staff were positive . The site was clean, organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and injury reports. Standard #: 22VAC40-185-160-C
Description: Based on staff record review, the center did not obtain from all staff results of a follow up tuberculosis screening at least every two years. Evidence: 1. Staff #3 began employment on 11/04/2013. Her last tuberculosis screening document was dated 4/28/2017 and had been more than 2 years.

Plan of Correction: The staff member was sent today to get the tuberculosis screening updated.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center outside were not maintained in a safe and operable condition. Evidence: 1. In classroom 105, the paint is chipping in between the two closets and a cabinet to the right of the sink. 2. In classroom 115, the paint is chipping near the tape on the wall. 3. In the classroom 119, the paint is chipping in the bathroom. 4. In the classroom 121, the drywall is cracked near the science table and the paint is chipping to the left of the window. 5. In the classroom 138, the paint is chipping near the family tree on the wall.

Plan of Correction: Facilities will be contacted to patch and repaint.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In Classroom 143, the cabinet above the sink contained 2 cleaning products and was in the locked position, but the key was in the lock. The teachers and children were not in the classroom at the time.

Plan of Correction: Staff were reminded that key can not remain in the lock.
