Comments:
An unannounced monitoring inspection was conducted with the director beginning at 9:30 AM and ending at 12:30 PM. There were forty-four children, ages six months through five years old, in care during the inspection. The children were observed being read a story by staff, coloring, playing in "home living" and with blocks. The infants were observed during outside recreational time. Ten children's records were reviewed and in compliance. Five staff records were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency evacuation log was reviewed and in compliance. The most recent evacuation drill was documented as 4/30/19. The most recent shelter-in-place drill was documented as 4/19/19. The most recent health inspection was dated 1/4/19. The most recent fire inspection was dated 1/29/19. Violations:
Standard #: 22VAC40-185-570-E
Description: Based on observation, the center did not ensure that all of the bottles were dated and labeled with the child's name. Evidence: In the infant room's refrigerator there were four bottles that were not dated and two bottles that were not dated and labeled with the child's name.

Plan of Correction: The staff will be retrained regarding labeling and dating all the infant's bottles.

Standard #: 22VAC40-191-40-C-1-B
Description: Based on a review of five staff records and staff interview, the center failed to obtain central registry reports for all staff within 30 days of employment. Evidence: Staff A (start date 3/28/19) and Staff B (start date 3/3/19) did not have documentation of a central registry report in their files.

Plan of Correction: The Central Registry Reports have been requested for the staff members but the results have not been received. The center has followed up with the Division of Social Services regarding the reports and will place the reports in the records once they are received.
