Comments:

A Subsidy Health and Safety inspection was conducted on April 30, 2019 to review supplemental health and safety requirements. Three staff were present with twenty-two children during the initial tour of the program. Additional children and staff arrived during inspection. Five children's records and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description:	Based on review and interview, facility did not ensure there is a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential in each room or area where children are present. Evidence: On April 30, 2019, the record for child #3 had documentation of a food allergy that was not noted on the allergy list. The allergy list was not dated.

Description:	Based on review, facility did not ensure 1 of 5 staff is certified in first aid training within 90 days of beginning employment. Evidence: On April 30, 2019, the record for staff #4 (start date May 22, 2018) did not have documentation of current first aid certification.

Description:	Based on review and interview, facility did not ensure required policies are developed, implemented and written. Evidence: On April 30, 2019, facility did not have a written policy and procedure that describes how they will ensure that each group of children receives care by consistent staff or team of staff members.

Description:	Based on review and interview, facility did not ensure the written emergency preparedness plan has documentation of all required procedural components. Evidence: On April 30, 2019, the emergency preparedness plan did not have procedures to address continuity of operations procedures to ensure that essential functions are maintained during an emergency.
