Technical Assistance:
Discussed using potting soil as sensory, tracking of children, using bathroom as storage, where to find additional staff trainings.

Comments:
An unannounced mandated Monitoring inspection was conducted today from 9:00am to 1:00pm. There were 33 children present with 5 staff. Children were observed having breakfast (cereal, mandarin oranges, and milk), painting, cutting with scissors, playing with blocks, reading a book, and making shapes with string. Reviewed 6 staff files, 5 children's files, fire and health certificate, first aid kit, attendance, fire and health inspections, and physical plant. Violations:
Standard #: 22VAC40-185-210-A
Description: Based on staff file review, staff files were incomplete. Evidence: Staff#1 and Staff#2, who were being utilized in the Program Leader role, did not have documentation of Program Leader qualifications available for review.

Plan of Correction: I will review older files to obtain documentation for Program Leader. If documentation can not be found, a Fast Track to Program Leader will be implemented.

Standard #: 22VAC40-185-260-A
Description: Based on file review, the center was not able to provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: Last file inspection on file was conducted 3/22/18, which is expired.

Plan of Correction: I called them today to start the process of getting inspected.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not being maintained in a safe and operable condition. Evidence: 1. The multi-colored Little Tykes dual slide on the Preschool playground is leaning. Slides and leg supports of structure have been compromised due to resilient surfacing pressure causing the structure not to be level. This violation was previously cited on 1/3/19. 2. The emergency egress door in the Pre-school room is not easily opened. Inspector had to push her whole weight on the door to open. Interview with Staff#1 revealed it is extremely hard to open. 3. Observed a skylight in the center hallway to have cracked and peeling drywall, as well as a dark colored substance suspected to be mold on the ceiling. 4. Observed a cabinet with broken plastic covering and exposed particle board on the side of the hand sink in the School age room. 5. An operating hot water urn and it's cord was being utilized in the hallway accessible to children. 6. Clogged Rain gutters were causing standing water pools in emergency evacuation routes, and on the playgrounds.

Plan of Correction: For all violations listed the plan of correction is to put in a work order by end of business today, and then follow up to ensure they get done.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not being kept in a locked place using a safe locking method that prevents access by children. Evidence: A supply closet in the center hallway containing cans of spray paint and other hazardous items was unlocked and the key left in the door.

Plan of Correction: The key is stuck in the door so a work order will need to be made. We will remove any hazardous items until it's fixed.

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient surfacing does not comply with minimum safety standards. Evidence: The resilient material on the Preschool playground has become compacted and washed away, and therefore does not meet the minimum standards of 6 inches in depth in fall zones. This violation was previously cited on 1/3/19

Plan of Correction: I put in a work order and will call to follow up

Standard #: 22VAC40-80-120-E-2
Description: Based on file review, the licensee did not obtain annually an inspection report from state or local health authorities that shall include approval of general sanitation and, if applicable, water supply, sewage disposal systems, and food service operations for the building or buildings in which the facility is operated. Evidence: The last health inspection of file was 4/16/18, which is expired.

Plan of Correction: Called and left a message for the health department
