Discussed the mats in the Toddler classroom.

Comments:
An unannounced monitoring inspection was conducted from 9:20 am-1:20pm. At the time of entrance, 126 children were in care with 25 staff members present. Children were observed during diaper changes, snack time, washing hands, working on a computer, engaged in group activities in the fitness room, coloring, playing in various centers, reading books and outdoor play time. Interactions between the children and staff were positive .A selection of staff and children records, the physical space, evacuation drills, attendance records and medications were reviewed. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of 10 staff records, the center did not have documentation of a negative tuberculosis screening within 21 days after employing or within 12 months prior to hire. Evidence: 1. Staff #1 began employment on 3/14/2019 and did not have record of a negative tuberculosis screening within 21 days after employment. 2. Staff #2 began employment on 12/05/2018 and did not have record of a negative tuberculosis screening within 21 days after employment.

Plan of Correction: We will send them to get that done either today or tomorrow.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain updated tuberculosis tests or screenings for all staff members every 2 years. Evidence: 1. Staff #9 had a TB test result in the file dated 9/21/2016 and had been more than 2 years since the last screening.

Plan of Correction: We will ask her to set up an appointment to get it done.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center were not maintained in a safe and operable condition. Evidence: 1. In classroom 4 Bear Cubs, a rug had a large tear on the edge of the rug. 2. The playground had an approximately 10 inch crack in the surface and about 4 other small cracks in other areas. 3. In the classroom 2 Ducklings, the paint is chipping near the window/diaper changing area.

Plan of Correction: The rug was removed immediately during the inspection. I will fill out the maintenance request for the paint chipping. I will investigate who I need to call to repair the surface.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff record review, the center did not have the central registry findings within 30 days of employment of all staff. Evidence: 1. Staff #1(date of hire 3/14/2019), Staff #2 (date of hire 4/08/2019), Staff #4 (date of hire 12/05/2018) and Staff #10 (date of hire 2/17/2019) have been employed with the center for more than 30 days and did not have the central registry findings.

Plan of Correction: I will follow up with them today and possibly need to resend them.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of 10 staff records, the center did not obtain fingerprint results for all staff members. Evidence: 1. Staff #3 (date of hire 09/08/2017) and Staff #6 (date of hire 06/01/2017) did not have the required fingerprint results.

Plan of Correction: I will have them schedule appointments to get their fingerprints completed.
