Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:35am - 12:00pm. There were 84 children in care with 16 staff. The children were observed painting, coloring, at circle time, listening to a story, playing on the playground and during lunch. Six staff's records and nine children's records were reviewed. One violation from the previous inspection had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a safe condition. Evidence: 1. The inspector observed the preschool playground. There are tree roots from the tree closest to the building that are exposed and creating tripping hazards. Also, on this playground there is a drain pipe with an extended cover that is cracked with sharp edges. The drain pipe is located next to the PS two classroom.

Plan of Correction: The roots will be covered with mulch and the drain pipe extender will be removed.
