Comments:
An unannounced on-site renewal inspection was conducted on 05/09/2019 and a record review inspection was conducted on 05/15/2019. There were 8 children, ages 3 years-5 years, and 3 staff members present during the on-site inspection. There was also an addition support staff member and program volunteer present during the on-site inspection. The inspector reviewed 5 children's records and 3 medications during the on-site inspection. The inspector reviewed 3 staff records during the 05/15/2019 staff record review. The children were observed in the following activities during the on-site inspection: free choice activities/centers, participating in an art activity and having an afternoon snack. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed the license renewal process and medication requirements with the provider. The inspector arrived for the on-site inspection at 2:45 p.m. and departed at 4:43 p.m. and arrived at 1:15 p.m. and departed at 2:00 p.m. for the record review inspection. Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards. Evidence: The record for Child #1 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the addresses for both contacts listed were incomplete.

Plan of Correction: The information was requested from the parent and completed at pick up on 05/09/2019.

Standard #: 22VAC40-185-280-D
Description: Based on observations and discussion with the staff, the facility failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food. Evidence: The storage closet contained 3 unlabeled plastic containers of cleaning solutions (2 of which contained a purple liquid solution and 1 that contained a clear liquid solution), a container of Fabuloso, a container of Clorox Wipes and a container of Lysol Wipes on the same shelf as and on the shelf above the following items: single-use gloves, tissues, baby wipes, Q-Tips, and a bag of individual packages of graham crackers.

Plan of Correction: The storage closet was rearranged during the inspection to have cleaning materials on the bottom shelf.

Description: Based on observations and discussion with the staff, the facility failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents and shall not resemble food or beverage containers. Evidence: There were 3 clear, unlabeled plastic spray containers in the storage closet. Two of the containers contained a purple liquid solution and 1 contained a clear liquid solution. The staff stated that they were cleaning materials left over from a community service project that had taken place the previous weekend.

Plan of Correction: The bottles were thrown out during the inspection.
