- Technical Assistance:
A lot of the rubber mulch on the playground has washed away to the edges of the playground from extreme weather over the last several weeks. Management was encouraged to rake mulch back around the slides and other fall zones.
Today an unannounced Monitoring Inspection took place between the hours of 1:25pm and 3:05pm. At the start of the inspection there were a total of 11 children with 2 staff; all ratios were incompliance. Additionally, there is private kindergarten, 1st, 2nd and 3rd grade classes taking place in the same building. A total of 5 children?s files and 3 staff files were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, each child did not have a physical exam on record within the first 30 days of attendance. Evidence: Child #3 (date of enrollment: 12/3/2018) file does not contain a physical exam. Plan of Correction: The parents know that they need to bring this in. The child has a doctor appointment on Monday and the parents will bring a physical exam.
Standard #: 22VAC40-185-160-A Description: Based on record review and interview, the center did not obtain a negative tuberculosis (TB) screening for each staff person within the first 21 days of employment. Evidence: The record for Staff #1 (date of hire: 4/11/2017) did not contain a TB screening. Center management stated that Staff #1 is not usually in the classroom with the children, however she is in there today because another staff member had a doctor's appointment. Plan of Correction: She is only a volunteer, we won't use her in the classroom anymore.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview, the center did not keep a complete record for each staff person. Evidence: The record for Staff #1 (date of hire: 4/11/2017) contained documentation of an emergency contact and sworn disclosure statement only. Center management stated that Staff #1 is not usually in the classroom with the children, however she is in there today because another staff member had a doctor's appointment. Plan of Correction: She is only a volunteer, we won't use her in the classroom anymore.
