Comments:
The inspector conducted an unannounced mandated inspection from 10:00-12:30. There were 42 children in care with staff supervising at all times. The director, and the cook and other support staff were present. Children were observed playing outside, playing inside, and eating lunch. Five children's records and five staff records were also reviewed. The violation is identified on the violation notice. An exit interview was conducted with the director. Please call your inspector at 276-698-9981 with any questions.

Description: Based on observation, the center had an area outside that was not safe. Evidence: 1. The inspector observed 5-7 pieces of tread that had come loose on on a piece of wooden playground equipment.

Plan of Correction: The director said she would take up the tread.
