Comments:
An unannounced Licensing Inspection and a Subsidy Health and Safety inspection was also conducted this date to review supplemental health and safety requirements. The two inspections combined cover all subsidy health and safety requirements. A total of 26 children with 2 staff present today. Children were observed arriving for the program, attendance was taken and bathroom procedures were observed. A total of 6 children's records were reviewed. CPR/First Aid and MAT trainings were verified. Staff records to include background checks are reviewed annually at the administrative office. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. This inspection began at 2pm and ended at 3:40pm. Description: Based on review of children's records, 4 out of 6 children with diagnosed food allergies do not have a complete allergy care plan. Evidence: Child #2,3,4 and 5 have diagnosed food allergies. allergy care plans were not maintained for these four children.

Plan of Correction: We will obtain a completed allergy care plan from their pediatricians.
