Health and Safety inspection was conducted today to review supplemental health and safety requirements. There were 16 children in care with three staff. Children present ranged in age from two to eight years old. Children were observed in teacher and self-directed activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Standard #: 22VAC40-665-540-A Description: Based on record review and interview the vendor failed to ensure that staff submit documentation of tuberculosis screening prior to coming in contact with children. Evidence 1 - The record for staff 2 does not contain documentation of a tuberculosis screening. a. Staff 2 has a documented date of hire as May 5, 2019. b. Staff 2 was present providing direct to children. 2 - Staff 1 confirmed that there was not any documentation of tuberculosis screening available for review during the inspection. Plan of Correction: Center management will require staff 2 to provide documentation of tuberculosis screening as soon as possible.
Standard #: 22VAC40-665-770-B-5 Description: Based on review of procedures and interview the vendor failed to create a continuity of operations procedures that ensure essential functions are maintained during an emergency. Evidence: 1 - Review of the current emergency emergency procedures for the center does not include a continuity of operations plan. 2 - Staff 1 confirmed that there is not a continuity of operations procedure available for review today. Plan of Correction: Center management will create and implement a continuity of operations plan as required.
