An unannounced monitoring inspection was made on 04/26/19 in conjunction with Child Protective Services in response to a self reported alleged incident of physical abuse to a child by a staff person. Six staff members, the director, and the parent were interviewed on this date. Based on these interviews, there was no violation of the Standards related to the incident. The inspection was completed on 05/16/2019 and sent to the facility on that date.
