Comments:
An unannounced monitoring inspection was conducted today from 9:45am to 1:40pm. There were 110 children in care of 24 staff members. The children were engaged in a circle time activities, observed going outside, playing with manipulatives, eating lunch and taking naps. The lunch menu included chicken nuggets, corn, pineapples and milk. 6 staff and 6 children's records were reviewed. Emergency drill records were also reviewed. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain additional immunization records every six months for children under the age of two years. Evidence: 1. Child # 1's ( 11 months old) record indicated that the most recent immunization records were from August 2018. Additional immunization records were not obtained for Child # 1.

Plan of Correction: I will ask the parents to bring in additional immunization records.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain a current record for each child enrolled. Evidence: 1. Child # 2's first date of enrollment was 4/3/17. Child # 2's record was not updated annually. 2. Child # 3's first date of enrollment was 7/29/15. Child # 3's record was not updated annually. 3. Child # 4's first date of enrollment was 11/28/16. Child # 4's record was not updated annually.

Plan of Correction: I will have the parents update the records.

Standard #: 22VAC40-185-340-D
Description: Based on record review and staff interview, each grouping of children did not have at least staff member who meets the qualifications of a program leader. Evidence: 1. In the older toddler classroom ( 2 year olds) there were three staff members present. 2. Staff # 1 stated that the program leader for the classroom is on medical leave and will not be returning on June 3rd. The program leaders leave started in March 2019. Staff # 1 stated that the three staff members present have been working in the classroom since the program leader went on leave. 3. A review of records indicated the three staff members do not meet the qualifications for a program leader.

Plan of Correction: I will place a staff person that is qualified to be a lead teacher until the program leader returns from leave.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation, the center did not maintain the ratio of one staff member for every 5 children. Evidence: At 12:05pm, the young toddler room had 23 children with 4 staff members. One staff member had left the room to get lunch, leaving the classroom out of ratio.

Plan of Correction: Staff will be retrained.

Standard #: 22VAC40-185-350-E-3
Description: Based on observation, the center did not maintain one staff member for every eight children in the two year old classroom. Evidence: At approximately 10am, it was observed that the Older Toddler classroom ( children 2 years old) had 22 children. One of three staff members walked out of the classroom, leaving the other two staff members over ratio.

Plan of Correction: Staff will be retrained about ratios.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not obtain central registry findings for an employee within 30 days of employment. Evidence: Staff # 2's first day of employment was 4/30/18. The central registry form for Staff # 2 was submitted on 2/26/18. There was no documentation to show that a follow up was conducted. Central registry results were not obtained by the center within 30 days of employment.

Plan of Correction: I will submit a new form today and follow up with the inspector.
