Comments:
An unannounced monitoring inspection was conducted today from 11:15am-12:30pm. There were 22 children directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outdoors, listening to a presentation by a local police officer including a tour of the police vehicle and being dismissed to their parents. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-240-A
Description: Based on a review of five staff files, two staff files did not contain documentation that staff received the required training by the end of their first day of assuming job responsibilities. Evidence: The records for Staff 1 and Staff 4 did not contain documentation that the staff members received an orientation.

Plan of Correction: We will follow up with the staff members to ensure that the correct documents are in the files and that they have recieved an orientation.

Standard #: 22VAC40-185-330-B
Description: Based on observation, 2 slides in use did not have a fall zone that would encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: Two moveable slides on the playground were placed in such a way that the child would slide into other play structures.

Plan of Correction: We will move the slides so that there is ample room.

Standard #: 22VAC40-185-510-G
Description: Based on medication review, one child's prescription medication was not maintained with the original prescription label attached. Evidence: Child 2's medication was in the original container, but the pharmacy label with the child's name was not attached to the box.

Plan of Correction: I will speak with the parents about obtaining a new label.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication, medications with expired authorizations were not picked up by the parent of disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet within 14 days of the authorization expiration. Evidence: Child 4's medication authorization expired on 4/15/2019 and the medication was present in the facility on the date of inspection 5/16/2019.

Plan of Correction: I will talk to the parents to either return the medication or get an updated authorization.
