Comments:
An unannounced monitoring inspection was conducted today from 12:30pm-3:15pm. There were 42 children in the direct care and supervision of 8 staff. The first aid kit, emergency materials, physical plant, 2 emergency medications and evacuation drills were reviewed. A sample of 8 injury reports, 5 children's records, and 5 staff records were also reviewed. The infants were observed being playing and sleeping. The children age 2-4 years old were observed taking a nap in the designated nap room. Any violations are listed on the Violation Notice. Violations:
Standard #: 22VAC40-185-350-E-1
Description: Based on observation and staff interview, the ratio of one staff member for every four children in the infant room was not followed. Evidence: 1. Upon arrival, there was one staff member with 7 infants (age 8 weeks-16 months) in the infant room from 12:25-12:40pm. 2. The Director stated that there were two staff assigned to the infant room, however the other staff member was on her lunch break.

Plan of Correction: We will rearrange schedules to make sure that the ratio is always covered in the infant room.

Standard #: 22VAC40-185-440-E
Description: Based on observation, the center did not ensure that there was at least 12 inches of space between occupied cots. Evidence: 1. In the nap room for the 2's, 3's, and 4's there was not 12 inches of space between all occupied cots.

Plan of Correction: We will change the areas that the children nap in to be able to make sure there is 12 inches between each child's cot.

Standard #: 22VAC40-185-450-A
Description: Based on observation, all cots used by children during the designated rest period did not have linens consisting of a top and a bottom cover. Evidence: 1. There were 4 children observed with a top cover on them, but no bottom cover between the children and the cot.

Plan of Correction: We will make sure all children have a top and bottom cover during the nap time.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center did not ensure that prepared infant formula was dated and labeled with the child's name. Evidence: 1. 2 out of 3 bottles were observed in the refrigerator with the child's name on them, but they were not dated. 2. 1 out of 3 bottles were observed in the refrigerator with no name and no date.

Plan of Correction: We will make sure we label the bottles with name and date everyday before putting them in the refrigerator.
