An unannounced renewal inspection was conducted today from 2:40pm-4:45pm. There were 34 children (12yrs.) supervised by 4 direct care staff. The physical plant, 2 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed transitioning into SACC, eating snack, and participating in various group activities. Hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, 3 of 4 children's records did not contain documentation of all of the required information. Evidence: 1. Child #1's record did not contain documentation of the child's first day of attendance. 2. Child #3's (start date: 9/6/2016) record did not contain documentation of information for a second emergency contact persons. 3. Child #4's (start date: 8/28/2017) record did not contain documentation of dad's place of employment. Plan of Correction: I will add start dates on files today. I will have the parents fill in the missing information today.
Standard #: 22VAC40-185-510-A Description: Based on a review of medications, 3 of 5 prescription medications did not contain documentation of written authorization from the parent. Evidence: 1. Child #1, Child #2, and Child #3's medication authorization forms did not contain documentation of parent signatures. Plan of Correction: I will have the parents sign the medication authorization forms today.
