Technical Assistance:
First aid/CPR and Virginia Preservice training requirement and deadlines for recently hired staff.

Comments:
A Subsidy Health and Safety inspection was conducted today from 10:30am-1:15pm to review supplemental health and safety requirements. 63 children were observed in the care of 13 staff. A sample of 10 children's records, 5 staff records, 10 injury reports, emergency drills, emergency supplies, medications, outdoor play equipment and other documents/areas were reviewed and inspected. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on records reviewed and interview, the department was not notified within 2 days of an injury to a child while under the center's supervision which resulted in a referral for treatment from a medical professional. Evidence: An injury report [date: 5/10/2019] documented that a child received a "deep cut" after bumping their head with another child. When asked by the licensing representative, an administrator disclosed that the child indeed received medical attention [at urgent care] following the incident. The department does not have a record of the incident being reported to the Division.

Plan of Correction: The licensing representative provided consultation on procedures for reporting serious injuries through the department's online system.
