Technical Assistance:
Discussed storage of personal items.

Comments:
An unannounced mandated Monitoring inspection from 8:45am to 11:00am. There were 43 children and 10 staff present. Children were observed reading books, having breakfast, dancing, singing songs, interacting with staff, having tummy time and playing with dolls. Reviewed 5 staff files, 5 children's files, emergency evacuation drills, attendance sheets, first aid supplies, medications and authorizations, and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, staff did not have documentation of being TB free within 21 calendar days of employment. Evidence: Staff#1, hire date 9/21/18, did not have documentation of a TB test within 21 days of employment.

Plan of Correction: Staff previously turned it in but it cannnot be located. Staff member will redo the tb.

Standard #: 22VAC40-185-60-A
Description: Based on record review, documentation of viewing proof of the child's identity and age was not obtained. Evidence: Child#1 did not have documentation that proof of their identity and age was viewed by the facility. Child was enrolled 8/28/2018.

Plan of Correction: I will have the parent bring in the birth certificate as soon as possible

Standard #: 22VAC40-185-280-H
Description: Based on observation, harmful agents were stored in areas accessible to children. Evidence: Observed a teacher's purse in Room B-2 on the floor of an unlocked closet which was accessible to children. This violation was corrected at time of inspection.

Plan of Correction: We will have staff use the locking cabinets in their rooms for purses.

Standard #: 22VAC40-185-350-E-3
Description: Based on staff interview and attendance record review, the center did not maintain a ratio of one staff member for every eight children. Evidence: At 8:50am, there was one staff with more than 8 two year olds. This was corrected at the time of the inspection.

Plan of Correction: We will make sure teachers tell us when they are at the ratio limit so we can send in staff to support before they are out of ratio
