63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today, May 16, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 12:30 pm and concluded the inspection at 3:00 pm. Upon the inspectors arrival, children were having lunch and preparing for nap time. Children provide their own meals which were labeled with names. Nap time was observed with soft music playing in the background. Children who were not sleeping looked at books while on their mat. Later elementary children were observed working on spelling skill outside under the canopy, playing a game of map bingo, and painting props for the end of the year play. Primary classrooms were observed with children creating books about the parts of a flower, matching activities, and grading skills using blocks. Children also had time outside on the playground. All areas of the facility were inspected. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, allergy information and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The inspector interacted with staff and children in care. Five staff records and eight children's records were reviewed during today's inspection. Medications and authorizations were reviewed. Staff on site have current CPR, First aid, DHO and MAT training. The last evacuation drill was conducted on 05/08/2019. A shelter-in-place drill for 2019 was documented on 03/12/2019. A second shelter-in-place drill will need to be completed by the end of the year. Health Inspection: 12/12/2018 Fire Inspection: 10/30/2018 The following ratios were observed in each classroom: Primary I: (3-6 years) 20 children with 1 staff, during nap time. Primary II: (3-6 years) 20 children with 2 staff. Pre-Primary: (2.5-3 years) 7 children with 1 staff. Two year old class: 8 children with 1 staff. Lower & Middle Elementary: 34 children with 3 staff. Standard #: 22VAC40-185-40-J
Description: Based on review, the facility did not ensure injury prevention procedures were updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: The injury prevention procedures were last updated on 12/29/2017, exceeding the annual requirement.

Plan of Correction: Per the administrator: corrected, The injury prevention plan was updated/ reviewed today.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the facility did not obtain all required information for each staff record. Evidence: 1. The record for staff #4, hired 02/04/2019, had two references that were not dated. 2. It could not be determined if the two references for staff #4 had been obtained prior to hire as required.

Plan of Correction: Per the Administrator: I have updated the staff reference check forms to include the date.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the facility did not ensure where playground equipment is provided, resilient surfacing complied with the minimum safety standards. Evidence: A mound of dirt without resilient surfacing was observed within the fall zone around the climbing dome. The mound of dirt covered an area of approximately four feet by three feet.

Plan of Correction: Per the Administrator: We will move the dome away from the dirt mound and refresh the mulch.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of staff records and interview, the facility did not obtain satisfactory results of the fingerprint-based national criminal background check prior to hire. Evidence: 1. Staff #4, with a hire date of 02/04/2019, had documentation of the fingerprint national background check dated 02/07/2019. 2. The administrator stated staff #4 was hired prior to obtaining the fingerprint national background check results.

Plan of Correction: Per the administrator: We have amended the process to be sure that all new staff is not hired before fingerprint results are received.
