Technical Assistance:
We discussed the requirements for background checks, TB statement updates, injury records, and staff records. The risk assessments for violations were provided.

We reviewed the requirements for allowable snack components, staff positioning on the preschool playground, infant formula requirements, and hand washing for infants/young children in diapers.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 10:03 AM to 4:00 PM. Today, there were fifty-four children, ages infancy through five years, with eight staff. I viewed program activities, equipment, staffing, supervision, interactions with children, infant care and records, diaper ointment, sunscreen, menu/nutrition, injury records, emergency supplies and drill records, five records for children, nine staff records, staff qualifications, staff training, background checks for two business representatives, agency inspection reports, insurance, posted information, indoor and outdoor areas. I viewed the changes for the space use and grouping of children for ages three through five years. The children have enjoyed frequent opportunities for active play outside. The staff were observed responding to children's needs for care and attention. The staff have planned age appropriate learning opportunities, such as colors and shapes for the youngest children and activities about various cultures for the older children. Issuance of the renewed license will be recommended pending receipt of a copy of the Articles of Incorporation to complete the application. Let me know if you have any questions. 540/430-9259

Standard #: 22VAC40-185-160-C
Description: Based on a review of eight records for staff, and an interview with administrative staff, the center failed to obtain updated tuberculosis screenings and statements at least every two years by the dates of the previous statements for three staff. Evidence: Staff member 2's TB statement completed on January 10, 2017 was not updated until March 11, 2019. The TB statement completed on January 10, 2017 for staff member 3 was not updated until March 11, 2019. The TB statement completed on August 1, 2016 for staff member 4 was not updated until April 1, 2019.

Plan of Correction: The director will use a system for tracking the dates of staff records so that arrangements for updating TB screenings and statements are made in time to obtain the statements by two two years since the last information was obtained. If the local health department is used for obtaining TB screenings, staff will be required to make appointments weeks in advance so that the screenings are not conducted late.

Standard #: 22VAC40-185-70-A
Description: Based on a review of eight records for staff, and an interview with administrative staff, the center failed to maintain information on the highest level of education for one staff member to demonstrate the qualifications were met for the position held. Evidence: A copy of the high school diploma or transcripts was not located for staff member 4 who was assigned as a lead teacher.

Plan of Correction: The director will arrange for staff member 4 to submit either a copy of a high school diploma or transcripts for the staff record.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury records from recent months, and interviews with staff, there was required documentation missing from the records. Evidence: Injury records viewed for April 1 and April 8, 2019 did not have information about the date, time and method of parent notification of minor injuries. Two injury reports completed on May 11, 2019 did not have documentation of the method of parent notification for minor injuries sustained by a child.

Plan of Correction: The staff immediately added information about parent notification to the four records. The director and assigned staff will review the injury records in the binders, and at the time injuries are treated, to make sure they contain all required information. The director spoke with the staff about using only the updated injury report form and completing all areas of the form. The director will review the required injury report information listed in the licensing standards during the next staff meeting.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of eight records for staff, and an interview with administrative staff, the center failed to maintain a copy of the Virginia Central Registry finding that was required within thirty days of employment for one staff member. Evidence: Staff member 8 began employment in 2015. The record contained a request form completed by staff member 8 for the central registry check that had two stamp in dates in red ink by the Virginia Department of Social Services agency in Richmond on April 15, 2015. However, the section for information about the completed search and finding by the agency was blank. A form with the finding was not located.

Plan of Correction: The director will have staff member 8 complete another request form for the Virginia Central Registry check and make sure it is mailed within seven business days. The finding will be obtained, viewed and filed in the staff record. The director will include summer workers and substitute teachers in the charted dates for record tracking. A copy of the results for staff member 8 will be shared with the licensing inspector.
