An unannounced Monitoring inspection was conducted in the center on May 16, 2019. There were 24 children present, the children were observed in two classrooms. Each group was supervised by two staff members. The ages of the children ranged from 3 to 5 years. As reported, there are 36 children currently enrolled in the program. The children were observed during nap time, free play, afternoon snack and dismissal at the end of the program day. A sample of 5 children's records was reviewed. Documentation of the fire and health inspections was reviewed. A discussion was held with the Center Lead regarding documentation of the fire inspection. As reported, the fire inspection was recently completed and documentation has been requested from the school office. Staff will continue to follow-up and will retain a copy for review. Complete staff records are maintained at the central office, the Licensing Inspector reviewed 4 partial staff records. Findings were reviewed with staff at exit. Standard #: 22VAC40-185-60-A
Description: Based on a sample review of children's records, the center failed to ensure information was updated at the time a child transferred from one licensed center to another. The following required information reviewed in the record was no longer current with the present enrollment: first date of attendance, agreement between the parent and center and previous child day care. Evidence: Information was not current in one of five reviewed records, the record of child #4. noted no previous child day care and first day of attendance as 10/23/18 and agreement was signed 09/21/18. The child enrolled in this center in April, 2019.

Plan of Correction: Program procedures will be reviewed to ensure staff are informed to document transfer information in the records as applicable. The child's record will be updated.

Standard #: 22VAC40-185-270-A
Description: Based on direct observations, the Center failed to ensure outside areas were maintained in a clean, safe and operable condition. Evidence: 1. The bottom of the fence gate leading into the playground had wire across the bottom that had pulled loose from the gate frame. A wire approximately 7 inches long with a sharp point protruded out from the fence at the ground level. Wire was pulled loose in several areas of the gate. A previous violation was cited on the January, 2019 inspection report. 2. Several areas of the fence had tree branches sticking through the fence or tree branches leaning over the fence into the path of the children on the playground and were within reach of the children.

Plan of Correction: A maintenance order has been submitted, staff will follow-up to ensure work is completed.
