Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the documentation of reviews for each child's emergency information.

Comments:
The following inspection took place between the hours of 10 am through 3 pm. Today, I met with the Staff of the Goddard School located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 120 children with 18 classroom staff; good ratios in all 9 rooms. A total of 10 children?s files and 10 staff files were reviewed. The children were engaged in the following: Tummy time, diapering, hand washing, infant feeding, art, puppets, large motor skills outside on center playground, computers, letters, center activities within the classroom, story time, shapes, song, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B
Description: Based on review, 1 of 10 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence - 1. On the date of inspection (05/16/2019), the immunization page on file for Child #3 was dated 10/04/2018. 2. On the date of inspection, Child #3 was under the age of two years.

Plan of Correction: An updated immunization page shall be obtained for Child #3 and placed on file.

Standard #: 22VAC40-185-140-A
Description: Based on review, 2 of 10 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance. Evidence - 1. On the date of inspection (05/16/2019), documentation of a physical for both Child #3, and Child #4 was not available for review. 2. The start date for Child #3 was listed as 01/04/2018. The start date for Child #4 was listed as 01/02/2019.

Plan of Correction: Current documentation of a physical examination shall be obtain for both Child #3, and Child #4 and then placed on file for each.

Standard #: 22VAC40-185-60-A
Description: Based on review, 4 of 10 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (05/16/2019) documentation of viewing the proof of each child's identity and age was not established for the following children: Child #1 (start date - 01/20/2019), Child #2 (start date - 05/06/2019), Child #4 (start date - 01/02/2019), and Child #10 (start date - 07/02/2019). 2. The Proof of Identification form for these listed children was either left blank or was missing.

Plan of Correction: The Proof of Identification form for all applicable children shall be completed. For all new children, this form shall be completed within 7 days of the child's original start date.

Standard #: 22VAC40-185-370-5
Description: Based on interview and observation, it could not be determined that infants, who could not turn themselves over and were awake, were placed on their stomachs for a total of 30 minutes daily in order to facilitate upper body strength and to address misshapen head concerns. Evidence - 1. On the date of inspection (05/16/2019), tummy time was not be recorded for those applicable children within the Infant I room. 2. The Infant Staff person stated (during an interview conducted on the date of inspection) that she tries to provide tummy time to those applicable children, but the date and time of those provisions could not be verified. 3. It was determined that the i-pad did have the ability to record and not that tummy time was administered.

Plan of Correction: Tummy time shall be documented in order to provide a written note within each applicable child's daily log.

Standard #: 22VAC40-185-440-F
Description: Based on observation, rest mats with cushioning can no longer be completely sanitized on all sides before the use by another child. Evidence - 1. On the date of inspection (05/16/2019), rest mats located in the Elephant room were observed to show a split along the side seam line revealing the cushion underneath. 2. With this exposed area, the rest mat can no longer be sanitized on all sides.

Plan of Correction: These mats shall be removed and replaced.
