Technical Assistance:
We discussed the requirements for diapering procedures. The risk assessment for a violation was provided. We reviewed the requirements for sight and sound supervision during nap time, and adding school information to children's records. Also, you were shown the procedure for serious injury reporting.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 10:39 AM to 4:30 PM. Today, there were seventy-nine children, ages infancy through five years, with fourteen to fifteen staff. After school, thirteen children, ages five through ten years were supervised by one staff member. I viewed program activities, daily routines, infant care, infant records, staffing, supervision, interactions with children, equipment, posted information, agency reports, insurance, sunscreen, diaper ointment, nutrition, meals/snacks, medication, diapering procedures, injury records, emergency supplies and drill records, ten records for children, ten staff records, staff qualifications, staff training, indoor and outdoor areas. The teachers have implemented age appropriate learning activities for their groups. The children had access to a variety of toys and materials. Teachers were observed responding to individual children's need for comfort and attention. Systems were in place for maintaining written information and records.

Standard #: 22VAC40-185-500-B
Description: Based on observation of of diaper change procedures, and an interview with staff, disposal has not always been done in such a way that neither the staff member's hand nor the used diaper touches an exterior surface of the storage system during disposal. Evidence: After a diaper change in the toddler room, a staff member was seen hand lifting the lid of the disposal container to place a wet diaper, used wipes and gloves into the container and did not use the foot pedal opening mechanism.

Plan of Correction: Administrative staff will review the requirements for disposing used diapers, wipes and gloves by use of the foot pedal to open the container lid. Administrative staff will make sure disposal containers are available close to the changing tables so that staff have easy access to the foot pedal for disposal. Administrative staff and supervising staff will conduct periodic observations of diaper changes to make sure that all required diapering procedures are followed.
