An unannounced Monitoring Inspection was conducted today from 4:55pm to 5:35pm. There were 12 school-agers directly supervised by 2 staff. The physical plant, staff records were current from previous inspection visit, 5 children?s records, children's medications and medication authorization records are current from previous inspection visit, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in a group game, and finishing snack. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Evidence: Reviewed the posted inspection reports and found that the most recent inspection dated was 3/6/18. The facility's last inspection was on 4/24/19.
