Comments:
A Subsidy Health and Safety inspection was conducted on Wednesday, May 15, 2019 from approximately 9:37 pm to 12:30 pm to review supplemental health and safety requirements. During this inspection, the center's director and owner were present and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 22 children with 5 staff present. During this inspection, the children in care were observed by the licensing administrator and licensing inspector completing a variety of activities to include engaging in outdoor play at the playground area, sitting at the table playing with Play Doh with staff, sleeping in crib, diapering by staff, participating in Yoga in the classroom, completing bathroom breaks, preparing for lunch time, and eating their lunch. The center's lunch for today consisted of chicken nuggets, peas, pears and peaches, and milk. The center's bathrooms, kitchen, designated classrooms, indoor areas, playground areas, and equipment were inspected by the licensing inspector. The required posting were reviewed by the inspector. The following information was reviewed: daily attendance, emergency supplies, evacuation and shelter-in-place drills, liability insurance policy, and allergy list. There were five children?s records and six staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Medication is not being administered at this time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on policy review and interview with staff, the center did not ensure to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: During the inspection on May 15, 2019, there was no written policy and procedures found to address the consistent care policy. Interview with staff confirmed a written policy and procedure

Plan of Correction: The center's owner will ensure a written detailed policy and procedures for consistent care policy is developed and reviewed with staff.

Description: Based on review of emergency response plan and interview with staff, the center did not ensure to develop a continuity of operations procedures. Evidence: During the inspection on May 15, 2019, a review of emergency response plan did not include a continuity of operations procedures. Interview with staff revealed the updates to the emergency response plan but it did not reflect the continuity of operations to address the essential functions.

Plan of Correction: The center's owner will develop a list to include the essential functions for the continuity of operations procedures and update the emergency response plan.
