An unannounced monitoring visit and facility tour was conducted from 9:55am-11:50am. At the time of inspectors' arrival there were 33 preschool aged children in care with 4 staff members and 1 administrator. Children were observed participating in snack, free play, and outdoor play. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection.
Standard #: 22VAC40-185-160-C Description: Based on record review it was determined that the center did not ensure that all staff resubmit TB test results at least every 2 years. Evidence: 1. The record for staff #1(hired 6/2/14) did not contain current TB test results. The results were dated 6/2016. 2. The record for staff #2(hired 9/1/11) did not contain current TB test results. The results were dated 5/12/17. 3. The record for staff #3(hired 9/1/89) did not contain current TB test results. The results were dated 8/26/16. Plan of Correction: -
Standard #: 22VAC40-185-280-B Description: Based on observation it was determined that the center did not ensure all hazardous substances were kept in a locked place using safe locking method that prevents access by children. Evidence: 1. The closet in the PreK4B room was unlocked and contained Lysol spray, Lysol wipes, Windex, and Dry Erase board cleaners. 2. The teacher confirmed that they did not lock the closet back after using supplies out of it. Plan of Correction: -
Standard #: 22VAC40-185-330-B Description: Based on measurement it was determined that the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: 1. There was 1" of mulch in the fall zone of the rock wall where 6" is required. 2. There was 0" of mulch in the fall zones of the slides. Plan of Correction: -
