Comments:
An unannounced monitoring visit and facility tour was conducted from 11:30am-12:45pm. At the time of inspectors' arrival there were 14 preschool aged children in care with 2 staff members. Children were observed participating in indoor free play, instructional time in the classroom, handwashing, lunch and transitioning between activities. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. There were no violations cited during this inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. There was worn and rusted links on the chains on the 6 swings utilized by children in care. 2. There was two rotting wooden boards on the play structures outside.

Plan of Correction: A maintenance request will be submitted 5/17/19. Maintenance will be completed prior to the beginning of the 19'-20' school year.
