Comments:
A subsidy health and safety inspection was conducted today with the Assistant Director who was present with 47 children and 9 staff. Two allergy plans were reviewed today. Please provide your plan of correction by 5/20/19. Arrival time: 10:30 am Departure time: 1: 40 pm

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on a review of six children's files and an interview with Administration, it was determined that there was no current written list of all children with allergies. Evidence: Child A enrolled on 4/30/19 with a diagnosed food allergy to milk, soy and nuts and the current allergy plan dated 11/18 did not have this child allergy listed.

Plan of Correction: Children have not been in attendance since beginning of May. Children will be dis enrolled upon confirmation of Social Worker.

Description: Based on a review of children's files and an interview with Administration, it was determined that child A had a diagnosed food allergy to soy, nuts and milk but did not have an allergy plan.

Plan of Correction: Allergy list has been updated and printed for proper placement.
