Comments:
An unannounced renewal inspection was conducted on 05/17/2019. There were 37 children, ages 5 years-11 years, and 6 staff members present during the inspection. The inspector reviewed 5 children's records and the on-site requirement for 6 staff records. The staff stated that there is no medication on site. The children were observed in the following activities: large motor play in the gym, having afternoon snack, in free choice activities and participating in an art project; a group of children were observed departing for a fieldtrip. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed behavior management, supervision and sanitizing with the staff. The inspector arrived for the inspection at 2:50 p.m. and departed at 5:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards. Evidence: 1. The record for Child #3 did not contain the health information as required by 22VAC40-185-130. 2. The record for Child #4 did not contain the health information as required by 22VAC40-185-140. 3. The record for Child #5 did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: The site director will get the missing information.

Standard #: 22VAC40-185-260-A
Description: Based on document review and discussion with the staff, the facility failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent fire inspection available for review during the inspection was dated 10/26/2017.

Plan of Correction: The site director will contact the school office as well as the corporate office.

Standard #: 22VAC40-185-280-B
Description: Based on observations and discussion with the staff, the facility failed to ensure that hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a spray bottle of Totally Awesome Degreaser and a spray bottle of Spic N Span in a milk crate sitting on the floor behind the staff desk. The cleaning materials were not locked and were accessible by children in the area that the unlocked cleaning materials were located.

Plan of Correction: The cleaning materials were locked during the inspection.
