Technical Assistance:
Consultation provided on maintaining accurate attendance logs as well as background check requirements.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with 51 children with 6 staff providing supervision. All of the children were napping during the walkthrough. Seven medications were reviewed as well as six children and five staff files. The center is up to date with their escape and shelter in place drills. Arrival time:1:10 pm Departure time: 3:30 pm

Standard #: 22VAC40-185-160-C
Description: Based on a review of five staff files, it was determined that one staff had not obtained a follow-up tuberculosis test/screening every two years as required. Evidence: Staff A's tuberculosis test is dated 9/29/16.

Plan of Correction: The staff will get the test/screening.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground it was determined that there was insufficient resilient surfacing surrounding the play equipment. Evidence There was no pea gravel one foot in front of the long red slide on the composite green and red structure on the older playground.

Plan of Correction: We will rake the pea gravel to ensure at least 6 inches in the fall zones.

Standard #: 22VAC40-185-510-C
Description: Based on a review of seven medications for seven children, it was determined that one child did not have up-to-date medication authorization. Evidence: The duration of the medication, Auvi-Q Jr. for child B was in effect until 2/20/19. The medication authorization from the parent and physician has not been renewed.

Plan of Correction: I have requested this information previously and will once again request an up to date authorization.

Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that the two infant bottles for child A was not dated with today's date.

Plan of Correction: Labeled during the inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of five staff files and an interview with the director, it was determined that one staff did not have a centeral registry finding on file within 30 days of employment. Evidence: Staff B was hired on 3/11/19 and does not have a central registry finding on file.

Plan of Correction: I will contact central registry to check up on the status and if they haven't received the request I will resubmit. Staff B will not return to the center until we have obtained a satisfactory central registry report.
