An unannounced complaint investigation was initiated on 5/16/2019. The complaint investigation concerned allegations of staff supervision, care and protection of children and incident reporting. Staff interviews were conducted. Both staff and child records were reviewed. Written incident reports were verified. An observation of the playground and the Pony's classroom was completed. Staff written statements were reviewed. The preponderance of evidence does not support a valid finding with regards to staff supervision, care and protection of children and incident reporting. However, during the investigation, violations pertaining to confidentiality were substantiated. Thank you for your cooperation during this investigation. Standard #: 22VAC40-185-40-E Complaint related: No Description: Based on both staff and parent interviews, staff written statements and child record review, the licensee failed to ensure the Center's own policies were followed. Evidence: Staff #1 failed to follow the Center's own policy on confidentiality on 5/14/19 when they shared confidential information about Child #1 to Child #2's parent. The Center's confidentiality policy requires personal information about children and their families be kept securely in a file available only to those parties involved. Plan of Correction: Retraining of all Staff on confidentiality policy and procedures.
Standard #: 22VAC40-185-50-A Complaint related: No Description: Based on both staff and parent interviews and review of a child record, the confidentiality of one child's record was not maintained. Evidence: Staff #1 shared confidential information regarding Child #1 to the parents of Child #2. Plan of Correction: Staff #1 employment has been terminated.
