Comments:
An unannounced monitoring inspection was conducted on 5/21/2019. There were 68 children in attendance during the day program. The children during the day program were grouped in 4 classrooms. Each group was supervised by two staff members. The LI reviewed a sample of 6 children's records. Documentation of the annual fire and health inspections was current. The inspector arrived at 10 am and departed the program at 12:30 pm. The documentation shall reflect the total number of children and staff in the center whom participated in the drill. Other observed activities included nap time, snack, centers and dismissal at the end of the program day. Findings were reviewed with the Lead Teacher at exit. Thank you.

Standard #: 22VAC40-185-430-H
Description: Based on observation, the center failed to ensure that trampolines were used. Evidence: 1. Observed a trampoline on one of the preschool classrooms. Verified through staff interviews with staff # 1 the staff stated that the trampoline was used last week during a program with the children.

Plan of Correction: The staff was unaware of the standard. In the future the trampoline will not be used.

Standard #: 22VAC40-185-510-G
Description: Based on review of eight medications, the center failed to ensure that all prescription medications had the pharmacy label. Evidence: 1. Prescription Medication for child # 7 did not have the pharmacy label.

Plan of Correction: In the future all medication that requires a pharmacy label will have the label before coming into the center.

Standard #: 22VAC40-185-510-J
Description: Based on observation of medications, the center failed to ensure that all medications were locked. Evidence: 1. Medications were in three out of four classrooms. All medications were stored in the file cabinets in the classrooms. The file cabinets were not locked in all three classrooms. The teachers were able to pull the file drawer open without unlocking the cabinet. The medications were not locked.

Plan of Correction: The site director will have the staff to lock all cabinets and will monitor all medications.
