Standard #: 22VAC40-185-130-B Description: Based on record review, it was determined that the center did not obtain additional immunizations every six month for children under the age of two years. Evidence: Child#1's record contains an immunization record that was last updated on 7/31/2018. Child#4's record contains an immunization record that was last updated on 10/10/2018. Plan of Correction: We'll ask them to update the immunization records, I will review all the other files to ensure that all the children have an update on their immunization records.
Standard #: 22VAC40-185-60-A Description: Based on record review, it was determined that children's records were incomplete. Evidence: Child#2's record does not contain the father's work telephone number and the mother's place of employment and work telephone number. Child#3's record is missing the father's place of employment and work telephone number. Child#6's record is missing the father's work telephone number and the mother's place of employment and work telephone number. Plan of Correction: We will talk to the parents to obtain the information that we need.
Standard #: 22VAC40-185-280-H Description: Based on observation of the physical plant, it was determined that a staff member's purse containing medications and cosmetics was not kept in an area inaccessible to children. Evidence: In the preschool classroom, a staff member's purse was observed on top of a low shelf, unlocked and accessible to children. Plan of Correction: Corrected during the inspection. I reminded the staff member to place their purses in the locked closet.
