An unannounced monitoring inspection was conducted today from 10:30am-2:30pm. There were 49 children (infants-5yrs.) directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, napping, playing outside, and eating lunch. Today's lunch consisted of fish nuggets, white rice, corn, oranges and milk. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-130-B Description: Based on record review, it was determined that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child#3's record contains an immunization record that was last updated on 8/17/2018. Child#4's record contains an immunization record that was last updated on 1/4/2018. Child#5's record contains an immunization record that was last updated on 9/26/2018. Plan of Correction: We have called the parents to bring updated immunization records. One record was faxed during the inspection.
Standard #: 22VAC40-185-60-A Description: Based on record review, it was determined that children's records were incomplete. Evidence: Child#3's record is missing the parents' place of employment and work telephone number. Plan of Correction: We will contact the child's parents to get the needed information.
Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that areas and equipment of the center, inside and outside were not maintained in a safe and operable condition.
Evidence:
1. In the preschool classroom there is a section of the wall next to the paper towel dispenser that is damaged. A soap dispenser was taken off the wall leaving a small hole and damaged drywall around it. 2. On the preschool playground the rubber playground surface was observed missing approximately 4 inches of the material on the left side of the playground equipment. 3. On the preschool playground an area of approximately 4 inches in length of settling ground that presents a possible tripping hazard was observed next to the brick fence surrounding the A/C - heating units.Plan of Correction: We will call maintenance and I will call the person that is in charge of maintenance to fix the items that need to be fixed.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: Several cans of paint were observed inside a storage closet next to the school-age classroom unlocked and accessible to children. Plan of Correction: We will have maintenance fix the lock.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on staff record review, there is no documentation that Child Abuse and Neglect results were obtained for employees that have resided outside the state of Virginia within the last 5 years. Evidence: Staff#5 (DOH: 04/10/2017) has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained. Plan of Correction: We submitted the form but we will call the state's background check department to check about the results.
