The Licensing Inspector for the YMCA Afterschool Program (Axton Elementary) conducted an unannounced Monitor Inspection on May 21, 2019. Upon arrival at 3:15 PM there were a total of 15 school age children present under the direct care & supervision of three (3) caregivers. Adult to child ratios were in-compliance upon arrival and throughout the inspection. Three (3) staff records and 5 children records were reviewed. After school program administers medication & medication records were reviewed. LI Anderson observed children during arrival/departure procedures, bathroom/hand washing procedures, snack/meal served, group activities, self-initiated activities and homework/quiet time. Inspection concluded at 5:45 PM. Violations found during today?s inspection were reviewed with the Center Director during the exit interview. Standard #: 22VAC40-185-60-A Description: Based on record review, the Licensee failed to obtain the name, address, and phone number of two designated people to call in an emergency if a child's parent cannot be reached. EVIDENCE: Licensing Inspector reviewed 5 children records. Record for Child #1 did not include the name, address and phone number to contact if the parents cannot be reached. Plan of Correction: Parents will provide an additional emergency contact person and document the complete address and telephone number.
