Comments:
An unannounced monitoring inspection related to a self-reported incident of a child being out of sight and sound supervision was completed on 5/21/19. Two staff were interviewed. Based on the interviews, there was a violation cited related to the incident.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interviews, the center failed to ensure that children under 10 years of age were within actual sight and sound supervision of staff. Evidence: 1. On May 8, 2019 a three year old child was not within sight and sound supervision of any staff for approximately 30 seconds to one minute. The child left the group with four other children and a staff person while entering the cafeteria area and proceeded to the front lobby area of the center where a parent arriving to the center found the child. The approximate distance from the cafeteria to the front lobby is 50 feet.

Plan of Correction: The director has been training the staff on consistently doing head counts and double check the number of children being supervised. The director has also changed procedures for having one staff in front of the children and one in the back while transitioning to different locations.
